INSURANCE!
 

A student MUST have insurance to try-out and/or play a sport.

 

There is a company that offers insurance to students called Myers-Stevens.  They offer two types:

 

24/7 Accident Only - Covers 24/7 for any covered accident, anywhere (except tackle football)

 

Accident and Sickness - Covers 24/7 for any covered accident including sickness (except tackle football)

If you fax the enrollment form to the company using a credit card the insurance will become effective at 11:59pm on the day it is received.  Please request a Confirmation of Coverage when you send it.

 

Please note:  The company does offer the other two programs (Sports and Tackle Football) listed in the brochure to Lawndale High School.   Do NOT sign-up or pay for either of these two programs.
