Officials
This page is for all Officials that will be working at Lawndale High School.
We are working really hard to get you a check on the day that you work a game.
In order to do this you must submit a completed W-9 to the Athletic Director, Julie Piller, at least 2 weeks before the game that you will be officiating. You can find the form at the bottom of this page.

DO NOT send it to the District Office. The check is written at the school and must be on file at the school before the check will be written.
You must submit a new W-9 each school year. Once you submit a form to our school it will remain on file for other sports that you officiate.
Please understand if you do not send a W-9 ahead of time or there are last minute changes to a game you will not be paid on the day of the contest. It will take up to 3 weeks to get a check.
